Default 98 Dodge 1500, 318 Problems

Ok I am fishing for ideas here as my dads truck has a issue.I think serious.
It started skipping and I plugged the little scanner I have into it and it said random missfire, missfire on 3 and 7.
Well I changed those plugs and no change in running.
There is something rattleing in the y pipe.
It has hardly no power at all and moans very load from under the hood.
There is no engine rattle, only the rattling loose thing in the y pipe. The plugs looked fine.
I have not done anything else but will be working on it to see what is wrong. He needs to know if he needs to get another truck or if we can fix this 1.
I have questions but mostly need ideas as to what to check.
I am curious as the moaning would indicate compresion going into the air intake?
So a injector or intake valve could have broke or dropped?
It has a timeing chain I would think instead of a belt right?
